Access all of the required forms to register for our program by clicking on the buttons below. Please email them to Jordan Polk at [email protected] OR to Ellie Moore at [email protected]. You can also bring them in person to our Welcome Center.
Submission of forms does not guarantee a spot for care. Spots for care will NOT be held unless ALL of the required forms are submitted! Spots are limited and available upon a first come, first serve basis. Our Child Care Director will follow up with each enrollment submission by phone or by email.
