Access all of the required forms to register for our program by clicking on the buttons below. Please email them to Emma Rappe at [email protected] OR to Ellie Moore at [email protected]. You can also bring them in person to our Welcome Center.

Submission of forms does not guarantee a spot for care. Spots for care will NOT be held unless ALL of the required forms are submitted! Spots are limited and available upon a first come, first serve basis. Our School Age Childcare Director will follow up with each enrollment submission by phone or by email. For questions about registration, please contact Emma Rappe OR Ellie Moore.
